All programs are recommended for adults 18 years and older. Adult-age caregivers, family members, and others members of a support team are highly encouraged to attend.

Programs Offered
HomeMeds
An evidence-based, in-home, medication review and intervention that includes a computerized risk assessment and alert process, plus a pharmacist review and recommendation for improvement. A complement to other evidence-based programs that address patient readmission reduction, health self-management, care transitions or caregiver support.
SAVVY Caregiver
Savvy programs are designed to provide the most relevant dementia knowledge, skills, and mastery to support family members as they provide care for their relative or friend living with dementia. Savvy Caregiver programs have been proven to decrease family caregiver distress, burden, and depression, while increasing caregivers’ sense of competence and confidence in their care role.
Chronic Disease Self-Management
A community-based workshop to help people manage their ongoing health conditions. This interactive workshop is designed to improve individual’s self-management skills. Different subject matter is taught each week with an overall goal of improved health and fewer sick days.
Diabetes Self-Management Program
A community-based workshop to help people manage their diabetes. This workshop is recommended for adults with Type 2 Diabetes or are pre-diabetic. This interactive workshop is designed to improve individual’s A1C’s and reduce the complications associated with Diabetes.
Chronic Pain Self-Management Program
A community- based workshop to help people manage their chronic pain. This workshop is recommended for adults who are experiencing chronic pain. This interactive workshop is designed to decrease pain and increase vitality.
Cancer: Thriving & Surviving Program
A community-based education workshop to help cancer survivors live a full and active life. This workshop is recommended for adults who currently have or have had a previous diagnosis of cancer.
Programa de Manejo de la Diabetes
A community-based workshop to help Spanish-speaking adults manage their diabetes. This workshop is recommended for adults who have Type 2 Diabetes or are pre-diabetic. This interactive workshop is designed to improve individual’s A1C’s and reduce the complications associated with Diabetes.
Tomando Control de su Salud
A self-management education workshop for Spanish-speaking people with a variety of chronic health conditions. It aims to help participants stay active and engaged in their lives and builds confidence in managing their health.
Building Better Caregivers
An interactive, community-based workshop for family caregivers of people with Traumatic Brain Injury (TBI), Post-Traumatic Stress Disorder (PTSD), dementia such as Alzheimer’s, and any other conditions that affect memory. Classes provide mutual support and build participants’ confidence in their ability to manage caregiving tasks and maintain a fulfilling life.
Powerful Tools for Caregivers
Classes help caregivers take better care of themselves while caring for a friend or relative. This evidence-based education program offers caregivers self-care tools to reduce stress, manage emotions, and increase confidence in coping with caregiving demands. Originally developed for caregivers of adults with chronic conditions, the program has expanded to include parents of children with special needs
Living Well With Hypertension
A one time, 2.5 hour session for people who have been diagnosed with hypertension or high blood pressure. Participants of this workshop will learn how to better manage this condition. This is a fun and interactive class.
Stepping Up Your Nutrition
A one time, 2.5 hour session for people who are interested in improving their nutrition and strength. This session is designed to help people understand the connection between nutrition and preventing falls.
Stepping On 7-week Program
A 7-week program proven to reduce falls and build confidence in older adults. Strength and balance exercises are taught along with other strategies to prevent falls inside and outside the home.
Enhance Fitness Group Program
A low-cost, evidence-based group falls prevention and physical activity program developed specifically for older adults. Classes meet three times a week, an hour each session, providing social stimulation as well as physical benefits.
Program To Encourage Active And Rewarding Lives
A one-on-one evidence-based program designed to reduce depression in physically impaired and socially isolated individuals. The program is offered in 6 – 8 sessions over a 19-week period by a certified PEARLS counselor.
Tai Ji Quan
An evidence-based falls prevention program delivered in two, one-hour sessions each week for 24 weeks. The primary focus is to improve strength, balance and mobility and prevent falls in older adults.
