Your patient can receive support from an evidence-based curriculum geared toward building self-efficacy, problem solving and self-management skills. These workshops support and enhance medical interventions that you are currently using. As a result, your patients understand the importance of setting and achieving self-management goals as a part of their total care plan.

Stanford Developed Self-Management Programs – Living Well
A 6-week program appropriate for people with any type of chronic condition. Workshops are taught by peer leaders using highly scripted materials. Stanford University has 30 years of evidence documenting the efficacy of the programs. Classes include:

Building Better Caregivers
For people who are family caregivers of people with Traumatic Brain Injury (TBI), Post-Traumatic Stress Disorder (PTSD), dementia such as Alzheimer’s, and any other conditions that affect memory.

Originally developed to reach caregivers of adults with chronic conditions, Powerful Tools for Caregivers has expanded the program to include classes for parents of children with special needs.
